I hereby authorize DNA Systems to conduct a full diagnostic of my hardware. A non-refundable fee of $60 applies for standard service, to be completed within 24–48 hours. Should an emergency diagnosis be required, a non-refundable fee of $120 applies, with completion within 12 hours.
Please ensure you have all licensing documentation available for any licensed software installed on your system. Some repairs may require us to completely erase all system data. Without valid licenses, we may not be able to reinstall the software. Therefore, DNA Systems cannot guarantee that unlicensed software will function after repairs are completed.
Please retain all power adapters, unless specifically requested to leave them with the equipment.
DNA Systems can only back up user data such as pictures, documents, movies, & music in user profile accounts. We cannot back up or reinstall software programs without valid licensing due to licensing restrictions. Re-installation of software will only be possible if valid licenses are provided.
For iMac models from 2013 or newer, the screen must be removed to access internal components, including servicing any hardware. Due to the delicate nature of this process even with great care, there is a risk of damage, including potential cracking of the display. DNA Systems will not be held responsible for any accidental screen damage incurred during disassembly. If damage occurs, screen replacement may be arranged at the customer’s request and expense.
If Customer loses, forgets, or otherwise misplaces any Encryption Key (Microsoft Bitlocker & Apple) that is under Customer’s control and that is required to decrypt User Data, Customer acknowledges and agrees that Customer may permanently lose access to the encrypted User Data and will not hold DNA Systems liable.
If repaired equipment is not picked up or paid for within (60) days of repair completion or service refusal, it will be considered abandoned. DNA Systems reserves the right to dispose of or sell abandoned equipment in accordance with applicable laws to recover any outstanding charges. DNA Systems retains all statutory and lawful liens for unpaid balances.
